- Conditions for Receiving Uniforms
Pick up in person or have someone else pick up on your behalf.
Bring the following documents:
- Gown Reservation Form (Blue Form)
- Receipt (Received via email)
- A photo may be used instead.
Note:
- Upon receipt, please inspect the condition immediately.
- Any corrections can only be made on the day of collection.
